Outpatient Cardiac Rehabilitation
What is Cardiac Rehab?
We offer medically supervised long term program to improve the cardiovascular health in patients. The program is usually initiated 1-3 weeks after hospital discharge. The goal of the program is to reduce the risk of death from heart disease or the risk of other heart conditions and improve the health and quality of life for our patients.
The Cardiac Rehabilitation Program at Bourbon Community Hospital assists you with a customized plan based on your needs. The plan includes exercise, nutritional counseling and educational resources--provided in a supportive environment, during informal sessions.
Who Is Involved In My Care?
- Respiratory Therapist
- Registered Clinical Dietician
- Clinical Pharmacist
- Social Worker (if needed)
- Medical oversight by a board-certified cardiologist
Our program includes as many as 36 one-hour sessions held three days a week. Physical activity counseling and exercise training; nutritional counseling; management of hypertension, weight and diabetes; and smoking cessation are major components of risk-reduction strategies for patients in cardiac rehabilitation. Our goal is to help you understand and manage your condition so you can resume activities and maintain a healthy heart.

Patients who have experienced or more of the following conditions can enroll.

- CAD, angina
- heart failure
- after a heart procedure
- open heart surgery
- CABG surgery
- angioplasty
- stents
- valve replacements
- pacemaker
- defibrillator
Insurance
Most major insurance carriers cover Cardiac Rehabilitation Program at Bourbon Community Hospital. We will pre-certify your coverage with your carrier prior to your sessions so you will know what is covered.
Referrals
If your physician feels that you would benefit from the Cardiac Rehabilitation Program at Bourbon Community Hospital, he or she may refer you to the program.
